Use the form below to request a pick up for your shipment. After your request is submitted, a FedEx shipping label is generated, faxed to your facility and a pick up is scheduled (if indicated). Put the label on the box and prepare it for shipping.

*If scheduling a pick up, it must be submitted before 1:00 pm central time to ship same day. After 1:00 p.m. (cdt), it may ship the following business day.
